A custom image for a reset button?
This document discusses, in addition to those techniques, the question whether and when this is a useful thing to do, as well as the necessity and placement of a reset button.
Perhaps this is caused by the mistaken assumption that a reset button is an obligatory part of a form.
For a typical form, a reset button is rarely useful, since it is unlikely that the user wishes to clear all fields.

Reset button - Wikipedia, the free encyclopedia
The reset button would typically kick off a soft-boot, instructing the computer to go through the process of shutting down, which would clear memory and reset devices to their initialized state.
Pressing the reset button would be preferable to the power button, which could potentially leave a device in the middle of some operation and subject to defect.
In the latter case, many times it switches off the system instead of restarting it (the reset button is actually also the on/off button, but under normal circumstances, the button needs to be held for 2-4 seconds to turn the system off).

Reset button technique - Wikipedia, the free encyclopedia
Writer may also use the technique after long story arcs to bring a series back to it's orignal ideals.
This sort of reset button allows writers freedom from complicated story arcs, and allows them to focus on indiviual adventures for the series.
However, many writers of Star Trek preferred a reset button over a continuous "story arc", feeling that episodes which only were a piece of a larger narrative would not stand up on their own as well, and that a major twist or resolution mid-series would constitute jumping the shark.

Reset / Cancel Button Considered Harmful (Jakob Nielsen's Alertbox)
The Reset and Cancel buttons are the Web's attempt at mirroring these features, but users usually prefer to use the Back button instead when they want to escape from an undesired state.
Some small amount of wasted time is spent scanning the useless button and deciding which of the two buttons is the correct one.
Even if a user were to use a form frequently, the reset button would still not be necessary if the data was mostly the same from one use to the next.

I'm using option radio buttons, some text boxes, and a list to allow a user to define what criteria they want to run a report for.

Radio buttons are like checkboxes except that when several share the same control name, they are mutually exclusive: when one is switched "on", all others with the same name are switched "off".
