Given that Medina Lake is a reservoir that has one very major permit holder who is allowed to take 66,000 acre feet of water from the reservoir each year, an explanation of surface water rights in Texas may shed some light on what this means.
Along the Medina River and its tributaries, from the headwaters to Medina lake, there are 50 individual permits for amounts ranging from 2 acre feet per year to 120 afy, mostly for irrigation of fields, though some are for other uses.
Below the Diversion Dam and on to the San Antonio River, the water of the Medina River (and its tributaries) is diverted by 82 permit holders for amounts ranging from 5 afy to 7500 afy.

Unlike the San Antonio River, which is controlled and channelized for much of its length in Bexar County, the Medina flows relatively undisturbed through healthy and varied riparian habitat throughout its length.
Due to the general west-to-east flow of the Medina, it does not appear to be a major avenue for northbound migrants, but the tall trees and brushy undergrowth pull in a surprising variety of species.
Bear in mind that the flow of the Medina is controlled at the Medina Lake dam, and responds quickly to rain in the area.

 USGS Scientific Investigations Report 2004-5209
The principal objective of this present (2001–02) study was to estimate ground-water outflow (seepage) from Medina Lake, Diversion Lake, and from the Medina/Diversion Lake system through the calculation of water budgets representing steady-state conditions over as wide a range as possible in the stages of Medina and Diversion Lakes.
During the time that the stage in Medina Lake was greater than about 1,040 feet, Diversion Lake gained more water than it lost to the ground-water system and the rate of ground-water outflow from Medina Lake increased sharply while its stage was between about 1,043 and 1,045 feet.
When the stage in Medina Lake is at the spillway elevation of 1,064 feet, Diversion Lake appears to gain as much as 40 percent of the concurrent ground-water outflow from Medina Lake.
