The Rogers Commission Report was created by a Presidential Commission charged to investigate the Space Shuttle Challenger disaster on its 10th mission, STS-51-L.
Other key findings of the Rogers Commission were that the Shuttle had not been rated to fly in the temperatures of the launch but that that technical concern had been overridden by NASA management after a series of launch delays.
The report was published on June 9, 1986.

The commission said two overall failure scenarios were possible: a small leak could have developed at ignition that slowly grew to the point that flame erupted through the joint as photographs indicated some 58 seconds after blastoff.
The Rogers Commission concluded that a "well structured" management system with the emphasis on flight safety would have elevated the booster O-ring issue to the status it deserved and that NASA's decision-making process was clearly faulty.
The Rogers Commission found that astronaut training time was being cut back, that frequent and late payload changes disrupted flight planning and that a lack of spare parts was beginning to manifest itself in flight impacts at the time of the Challenger accident.

Rogers was originally a merchant, but in 1708 fellow Bristol merchants, whose ships were falling prey to pirates, sponsored a retaliatory global expedition and selected Rogers to command it, with William Dampier as his navigator.
Rogers' described his crews as "tinkers, taylors, hay-makers, pedlers, fidlers etc, one negro and about ten boys." His mission was to harass Spanish shipping; to the English he was a loyal citizen but to the Spanish he was a pirate.
Rogers received a group of representatives from Harbor Island who assured him that, unlike Vane, many of the pirates were eager to accept the King's amnesty.

Roger Boisjoly, a Thiokol engineer a nd probably the most outspoken internal critic of the launch, had this to say during the interview, "There was never one comment in favor, as I have said, of launching by any engineer or other nonmanagement person in the room before or after the caucus" (93).
Throug hout this chapter, the report is rather critical of NASA and Thiokol for their failure to acknowledge and deal with the well-documented O-ring and joint problems of the SRBs.

And one of the things that we discovered after this accident was many of the recommendations made by the Rogers Commission either hadn't been enacted at all by NASA or had been enacted and then allowed to erode over the years.
I think the Commission report is very strong on noting that since the end of the Apollo program we've continued to have a manned space flight program but no avowed, explicit national policy of why we're sending people into space.

Leaking gas was already known to occur by at least one Thiokol engineer, Roger Boisjoly (pronounced "boy-shzo-lay.") His analysis of photos from earlier launches proved that the hot gases went past the primary and secondary O-rings to the outside.
Roger Boisjoly's lawyers told him not to offer any information other than what was asked for, but he decided to talk freely anyway.
Roger tried to stop the launch, because he knew the effects of the extreme cold on the rubber O-rings.
