SMOM is considered to be the most direct successor to the medieval Knights Hospitaller, also known as the Knights of Malta, and today operates as a largely charitable and ceremonial organization.
The large passage fees (alleged in some cases to be in the region of $50,000) collected by the American Association of "SMOM" in the early 1950s seemed to have tempted a Charles Pichel to create his own "Sovereign Order of St John of Jerusalem, Knights Hospitaller" in 1956.
Pichel avoided the problems of being an imitation of "SMOM" by giving his organization a mythical history by claiming the American organization he led was founded within the Russian tradition of the Knights Hospitaller in 1908, a spurious claim, but which never-the-less misled many including some academics.

 Sovereign Military Order of Malta
In any of the two cases, I think that the SMOM should be considered as a country, as it have many of the functions of a country: it has diplomatic relations with many countries, issues passports (only countries, the Red Cross, and the UN issue passports), and is member of a few international organizations.
SMOM is not categorized by the UN as a non-member state (like Switzerland and the Holy See) but among "entities and intergovernmental organizations having received a standing invitation to participate as observers."
SMOM is a legitimate and highly praiseworthy organization and is widely recognized as possessing sovereign status (useful in fulfilling a nonbelligerent honest broker's role in wartime situations).

SMOM is sovereign and has diplomatic ties with a number of countries, but it has no territory except a few buildings used for administration.
Apparently the logic of the SMOM position is that the Order has always had sovereign status independent of whatever territories it may or may not have occupied from time to time.
It seems that SMOM (BTW, it is a subject to international law ruled by the prince, in other words, SMOM is a "state") has indeed two variant flags.

It's important to notice that the vehicles with SMOM plates are not the official vehicles of the Order of Malta because they have standard Italian diplomatic plates with the letters XA at the end (see table).
Indeed, the SMOM plates are issued by the Italian Minister of Defence, and so somehow they are part of the Italian license plate system, but anyway their shape is far different from any other Italian plate, as you can easily see in the pictures, and they are not totally Italian.
Another strange thing about SMOM plates: some plates seem to miss the dot after the second "M" as you can see in one of the forth picture (many thanks to Maik Knoll for the picture, even if the quality is poor).

As I was given to understand at the time, there was some kind of exotic, mysterious, postage-stamp-sized (and presumably, -issuing) principality named Smom, discreetly tucked away "in a building next door to a tailor shop in Rome." Not unlike the New York City headquarters shown in the opening of The Man From UNCLE, I pondered.
Smom is an observer-status member of the UN, maintains diplomatic relations with some 70 countries, and -- sure enough -- issues its own stamps, coins and passports.
So far I haven't been able to unearth a national anthem for Smom, though there is one for its ultra-big-sister state, the Vatican.

Nevertheless, as a concession to the rising political, economic, and military power of the United States, in 1927 SMOM agreed to incorporate an American National Association whose members were not obliged to prove their genealogical pedigree.
In 1949 SMOM published an Official General Roll of the Grand Magistery with a preface by Pius XII which referred to "The ancient laurels collected on the battle-fronts" of earlier wars.
SMOM continues to regard the Venerable Order with somewhat amused score; while SMOMs are members of the highest levels of the Venerable Order, there do not appear to be any Anglican members of the Venerable Order in the British Association of SMOM.

On November 17, 1948 SMOM awarded one of its highest honors, the Grand Cross of Merit, to Reinhard Gehlen, the Nazi chief of intelligence on the Soviet front.
As its name suggests, SMOM is both a ''sovereign'' and, historically, a "military" organization.
In 1571 SMOM's fleet participated in the defeat of the Turks at the naval battle of Lepanto, and remained a major military presence in the Mediterranean until 1789 when Napoleon defeated the Knights and occupied the island.

The Holy See is a member of the international postal union, unlike the S.M.O.M. (which nonetheless has signed many international postal agreements) and both use Italian currency.
Thus the same principles that are applied in recognizing the Sovereignty of the Holy See may be applied to the S.M.H.O.M., whether or not the latter actually possesses "Sovereign territory".
These are together grounds for its acceptance into membership of the United Nations as a voting member of the General Assembly.

SMOM gave its highest award of honor, the Gran Croci Al Merito Conplacca, to Gehlen in l948.
Gehlen, who was not a Catholic, was awarded the honor because of his efforts in the "crusade against godless Communism." Gehlen headed Adolf Hitler's spy operations against the Soviet Union during World War II.
The origin of the SMOM predates the Crusades which began in the twelfth century.

For a better co-ordination, it was proposed to set up an ECOM office in Pristina.
ECOM has set up a logistics centre in Skopje that collaborates with SMOM’s embassy and the local Caritas units for the distribution of humanitarian aid and to support the social centres of the local Caritas.
