SNMP was designed in the late 80's to facilitate the exchange of management information between networked devices, operating at the application layer of the ISO/OSI model.
SNMP is built around the concept of "managers" and "agents." Manager software (commonly installed on a network management system) makes requests to agent software running on a host or device to gather data on the operational status, configuration, or performance statistics of that system (polling).
SNMP uses 161/udp for general purpose (request/response) communications, and 162/udp for traps.

 PHP: SNMP Functions - Manual
I've found that when using SNMP on Windows (2000 Pro) the drive the mib files have to be located on actually depends on where you subsequently call PHP from.
Note: If you need SNMP whilst using apache you may have to use the PassEnv command with MIBDIRS in your apache config, I haven't tried this yet.
In addition to working on NT as the documentation says, SNMP works on XP if you have the snmp extensions enabled in the ph.ini file, point the ini file to the correct extensions folder, and copy the mibs from the full install package to c:\usr\mibs.
