The strategic deterrent patrols of the Regulus missile boats were not deemed worthy of this insignia.
The badge is considered a "secondary insignia", meaning that the badge is worn secondary to a primary warfare pin, such as the Submarine Warfare insignia.

The first SSBN was the USS GEORGE WASHINGTON (SSBN-598), which started it's first strategic deterrent patrol in 1960.
Every individual is authorized to wear the Deterrent Patrol insignia when onboard during the alert status for 24 hours or more, as a symbol of their service.
At the end of the patrol, the submarine comes off alert status, and usually takes on one or more inspection teams before their return to port.

During the cold war, the importance of maintaining nuclear powered ballistic submarines on deterrent patrol was recognized and on 28 Jan 1969 a pin was issued to denote patrols made.
The pin was for the first patrol, gold stars for subsequent patrols, and silver stars for 5 patrols (the first silver star replaced 4 gold and the implication that the pin itself represented one patrol).
The Neptune award is given to all sailors completing 20 patrols and special recognition is given to the American sailor with the most SSBN patrols.

During his second patrol the ship fired three Polaris missiles on his birthday, giving him big candles that did not fit on his cake.
To complete this tour he was selected for the Limited Duty Officer Program and on 2 April 1979 he was commissioned an Ensign.
