 N-BASE - Submarine accident list
18 August 1959 : A helicopter engine exploded on board the U.S. aircraft carrier USS Wasp (CVS-18) which was operating 250 miles off Norfolk, Virginia.
The plane burst into flames and crashed into a nuclear bomb storage igloo in which there were three Mark 6 nuclear bombs.
Also, seven nuclear reactors (5 Soviet and 2 U.S.) from three Soviet and two U.S. nuclear-powered submarines have been lost at sea due to accidents.

In 1991, 200 missiles were thought to be in service aboard 14 Sovremenny destroyers and 18 Tarantul III missile corvettes.

 R-29 / SS-N-8 SAWFLY Mod SLBM - Russian / Soviet Nuclear Forces
Further tests took place from August through November 1972, during which 18 out of 19 launches were successfully conducted.
The R-29 missiles and the D-9 launch system were made operational on March 12, 1974 and they are deployed on 18 Delta I submarines.
The Delta Class nuclear submarine can launch missiles at approximately 7-second intervals while fully submerged.

On 18 February 2004 Russia launched four missiles as part of its ongoing military exercise, but one of them was self-destructed after veering off course.
A Molnia communication satellite and an intercontinental ballistic missile were launched from Plesetsk cosmodrome, a strategic RS-18 (Stiletto) missile was launched from Baikonur cosmodrome.
The new missile's length was increased only slightly, from 14.1 meters to 14.8 meters, allowing the overall dimensions of the launching tube to remain constant.

SS-18 Mod 1 - R-36M The SS-18 Mod 1 carried a single large reentry vehicle, with a warhead yield of 18 to 25 MT, a distance of about 6,000 nm.
The first three regiments were put on alert on 18 September 1979.
In November 1979 the flight tests of the MIRVed missile were completed.

 Submarine-Launched Ballistic Missiles
2001 Sept. 18: The long-range ballistic missile blasted off from the Podolsk submarine in the Sea of Okhotsk and successfully reached the target in the Barents Sea, navy announced.
By mid-March 2004, Russian media reported that a special commission investigating the mishaps during Security 2004 exercise, concluded that the failure of the ADK-3M/Sluz automated launch system caused aborts of missile firings on February 17.
The missile lifted off successfully, however after 98 seconds in flight it deviated from the assigned trajectory and self-destructed, according to Igor Dygalo, chief of the Russian Navy's press center.

 Submarine USS R-18 (SS-95) was authorized to be built by the
USS R-18 (SS-95) departed Hawaii on 12 December 1930; transited the Panama Canal; then continued north through the Caribbean and on up the east coast of the United States to the Philadelphia Navy Yard at Philadelphia, Pennsylvania.
When commissioned, the R-1 Class coastal and harbor defense submarine was 186'2" in length overall; had an extreme beam of 18'; had a normal surface displacement of 569 tons, and, when in that condition, had a mean draft of 14'6".
Following shakedown, USS R-18 was assigned to patrol the water approaches to the Panama Canal during the final months of First World War fighting...which ended on 11 November 1918 when the terms of the Armistice went into effect.

Transferred to the Pacific fleet in 1930, USS S-18 departed New London/Groton on 24 May 1930; operated off the California coast into the fall; and, arrived at her new home port, Pearl Harbor, in the Territory of Hawaii, on 7 December 1930.
In September of 1941, USS S-18 returned to the west coast of the United States; and, three months later, after the United States had entered World War II as an active participant, the submarine was ordered to the Aleutians.
