Although the extra features unrilated to screen reading can be nice and very handy in some ways, they also make screen readers more complicated to use and most importantly, make using computers very different in comparison to how sighted people do it.
I think the goal of the screen reader should be mostly in just reading the screen, leaving battling with the inaccessible quirks of a specific program and some of the interpretation of the user interface to the user (or for the plug-ins if people want that functionality).
