 BURNS SEQUELAE AND THEIR PLACE IN THE ACTIVITY OF OUR CLINIC
Our aim is to draw attention to the importance of preventing sequelae in the treatment of burns and of selecting the appropriate surgical procedure in cases where sequelae have already occurred.
Regarding the 263 patients operated on for burns sequelae, 120 (45.6%) were male and 143 (54.4%) female; 157 (59.8%) came from urban zones and 106 (20.2%) from rural zones of our country.
The distribution of sequelae according to age is shown in Table I. Regarding the different types of sequelac, we treated 98 contractures (37.3% of the operations), 73 scars (27.7%), 19 keloids (7.3%), 79 alopecias (11.0%), 13 ectropions (4.9%) and 31 trophic ulcers (11^).

Generally, the new problems associated with postpolio sequelae are gradual and non-traumatic, but acute injuries or events, such as herniated discs, or broken bones from falls, may be markers for establishing a disability onset date.

 THE USE OF TISSUE EXPANSION IN BURNS SEQUELAE
Controlled tissue expansion is a very worthwhile tool for the plastic surgeon who approaches the reconstruction of burns sequelae by covering the defects with adjacent donor tissues of similar colour, innervation, hair-bearing qualities and texture, leaving intact the adjacent skin structures.
This technique has the unique characteristic of providing us with extra tissue without leaving an unsightly donor site.The sequelae of most burn wounds involve a certain degree of contracture, even if wound healing is by epithclialization of a graft.
When we are faced with the reconstruction of postburn sequelae, we usually find a shortage of neighbouring donor tissues with similar tissue properties.

 Psychological Impact of Lightning Injury
The studies cited in Table 1 describe a variety of emotional problems, ranging from anxiety to marital break-down to major depression, and illustrate several main features of late psychologic sequelae: they are variable in severity and duration, they are difficult to predict from injury parameters, and their etiology is not readily inferred.
The role of secondary gain is no more or less prominent in the sequelae of EI and LI than in other instances of injury, and in a percentage of cases is likely serving to maintain chronicity, although it falls short of an adequate explanation for the host of symptomatology presented in the literature.
Data reviewed here suggest that a fourth group exists, a group of LI and EI survivors who maintain significant symptomatology beyond 3 years postinjury and per- haps for a lifetime.(24) This group may also be characterized by a greater degree of psychologic overlay in their presentation and level of dysfunction.
