  Serologic Tests
Testing for HCV RNA is a reliable way of demonstrating that hepatitis C infection is present and is the most specific test for infection.
Testing for HCV RNA by PCR is particularly useful when aminotransferases are normal or only slightly elevated, when anti-HCV is not present, or when several causes of liver disease are possible.
This test is useful for evaluation of patients who have abnormal liver function tests since it allows a direct quantification of multiple different serum proteins.

NDI Terminology - serologic test for syphilis   (Site not responding. Last check: 2007-10-10)
There are two types: nontreponemal antigen tests detect antibodies to substance (reagin) derived from host tissues, now known to consist of the phospholipids cardiolipin and lecithin; they originated with the Wassermann test and are now represented by the VDRL and RPR (rapid plasma reagin) tests.
pallidum immobilization) test and are now represented by the DFA-TP (direct fluorescent antibody–T. pallidum) test, the FTA-ABS (fluorescent treponemal antibody absorption) test, the MHA-TP (microhemagglutination assay–T. pallidum), and assays using ELISA (enzyme-linked immunosorbent assay) methods.
The term "serologic tests for syphilis" is occasionally used with reference only to nontreponemal antigen tests.

 2002 Treatment Guidelines -3
Serologic testing and a sexual history also should be obtained at 28 weeks of gestation and at delivery in communities and populations in which the risk for congenital syphilis is high.
Serologic testing of the mother's serum is preferred over testing infant serum, because the serologic tests performed on infant serum can be nonreactive if the mother's serologic test result is of low titer or if the mother was infected late in pregnancy.
All infants born to women who have reactive serologic tests for syphilis should be examined thoroughly for evidence of congenital syphilis (e.g., nonimmune hydrops, jaundice, hepatosplenomegaly, rhinitis, skin rash, and/or pseudoparalysis of an extremity).

 Practical Guidelines for Lyme Testing
Routine follow-up serologic tests to assess the efficacy of antiobiotic therapy are not routinely recommended, but may be of value in the evaluation of patients remaining symptomatic after treatment.
Since neither the PCR or serologic testing is 100% sensitive in the setting of early Lyme disease, combining the two may improve the diagnostic yield.
The positive test may be either a false positive or it may reflect residual antibody from a remote episode of Lyme disease independent of the patient's presentation now.

Serologic testing of the mother's serum is preferred to testing infant serum, because the serologic tests performed on infant serum can be nonreactive if the mother's serologic test result is of low titer or if the mother was infected late in pregnancy.
All infants born to seroreactive mothers should be evaluated with a quantitative nontreponemal serologic test (RPR or VDRL) performed on infant serum (i.e., umbilical cord blood might be contaminated with maternal blood and might yield a false-positive result).
Children who are identified as having reactive serologic tests for syphilis after the neonatal period (i.e., at greater than 1 month of age) should have maternal serology and records reviewed to assess whether the child has congenital or acquired syphilis (for acquired syphilis, see Primary and Secondary Syphilis and Latent Syphilis).

Although, ideally, all of these tests should be conducted for each patient who has a genital ulcer, use of such tests (other than a serologic test for syphilis) may be based on test availability and clinical or epidemiologic suspicion.
For communities and populations in which the prevalence of syphilis is high or for patients at high risk, serologic testing should be performed twice during the third trimester, at 28 weeks of gestation and at delivery.
Serologic titers may be checked monthly in women at high risk for reinfection or in geographic areas in which the prevalence of syphilis is high.

The purpose of testing for HIV must be to benefit patients, enable them to obtain care and counselling and make their behaviour safer for themselves and others.
Testing should always be done with the patient's informed consent after a full discussion of the consequences of being tested, including if, when and under what circumstances test results might be disclosed to others.
Inform the patient that test results and information will be added to his or her medical record and will be available to other health professionals on a need-to-know basis.

Serologic testing for FeLV is important in order to identify infected cats and prevent transmission of the virus (see Feline Leukemia Virus and Related Diseases: Introduction).
Antibody testing may be preferred because antibodies are produced to both male and female worms, and they are produced as early as 2 mo after infection, which is much earlier than antigen may be detected.
This test takes ~1hr and may be performed with zinc sulfate solutions made in the clinic, or reagents for the test may be purchased as a test kit.

Referral for liver biopsy should be considered only after a thorough, noninvasive serologic and radiographic evaluation has failed to confirm a diagnosis of cirrhosis; the benefit of biopsy outweighs the risk; and it is postulated that biopsy will have a favorable impact on the treatment of chronic liver disease.
The ALT is thought to be the most cost-effective screening test for identifying metabolic or drug-induced hepatic injury, but like other liver function tests, it is of limited use in predicting degree of inflammation and of no use in estimating severity of fibrosis.

Genital Infections   (Site not responding. Last check: 2007-10-10)
Serologic testing is by definition an indirect method of diagnosis since it relies upon a humoral immune response to infection.
Serologic tests — Because of lack of availability of equipment, test reagents, or the skilled personnel required for direct visualization techniques or PCR, a substantial proportion of patients suspected of having syphilis must be diagnosed by alternative methods.
The use of a single serologic test to diagnose syphilis is generally inadequate because of the potential for false-positive results [12].

 Herpes Diagnosis Facts
Serologic testing has the advantage that it can be done even when a person has no symptoms, so it is a very effective way to detect an established herpes infection.
Type-specific blood tests are ideal for those who have had a history of genital symptoms but have never had a successful confirmatory test.
Unlike the culture method, this test does not require growing the virus but rather seeks to identify herpes by the presence of antigens, fragments of the virus that are known to stimulate the immune response.

For example, the likelihood ratio of a positive ELISA test result in a patient with non-specific myalgias is 5.0 (five), which means that the likelihood of getting a positive ELISA test is 5 times greater in patients with Lyme disease than in patients without Lyme disease.
However, the probability calculator may be useful in dissuading a sole reliance on serologic testing to confirm a diagnosis of Lyme disease.
By using the calculator, the ACP Initiative on Lyme Disease hopes to discourage undue reliance on serologic testing and emphasize patient history and clinical judgment when evaluating patients with possible symptoms of Lyme disease.
