They can make those boxes so small and affordable (I won't lie and say they're cheap) because they have exactly one modulator circuit which is tuned to the lowest frequency channel in the band, and exactly one digital decoder, and exactly one demodulator which can be tuned to any of the incoming channels.

I think its because in the early days of set top boxes, TV sets had flat tops and (mock) wooden cases etc. This meant a box could happily sit on top of a TV without falling off.
Its called a Set-Top Box because it it in therory sits on top of your TV but as TVs are getting narrower and narrower they have to sit else where.
I think there was a thread on this last year, from what i remember we concluded that 80% of people did not have their "set top box" on top of their set.
