Shallow Hal is not so much about how gross people are as how beautiful they are once you get beyond the rude, noisy flesh.
Shallow Hal begs for the Farrellys to unleash their arsenal of offensiveness, but they want to be liked so much they appear afraid to offend.
Shallow Hal has none of that because the script is braindead and the plot is routine and predictable.

Hal deeply, traumatized by his father's death, grows up to be a relatively nice person who is only interested in the most physically perfect of women.
At the end of the movie Hal, predictably, starts to see Rosemary as she actually is rather then as he has been.
All of that is pretty mild compared to this guy who apparently was really turned off to the whole idea of Shallow Hal because he's really turned on by the idea of 300 pound women.

At age nine, Hal is told by his dying father that looks are all that matters when it comes to the female gender, and that he should only chase hot young tail.
Hal takes dad's advice and runs with it all the way up to his adult years, where he is still a pathetic loser trying to score with women completely out of his league.

In Shallow Hal, every character Hal encounters is bludgeoned into two stereotypical molds: all ugly and undesirable women are inherently GOOD (volunteers for children's hospitals or animal shelters or looking after their sick grandmas) and anyone with less than 10% body fat HAS to be "ugly inside".
Hal also initially pursues Rosemary because he is aroused pruriently by her sleek curves - there's no escaping this ultimate goal for a man ("seeing inner beauty" is always merely just one line of many we employ to achieve our goal of Laying That Pipe).
For those who argue that the rich Rosemary accepted Hal as her mate even though he was from a lower social caste, remember that SHE was the undesirable party, with low self-esteem, so any overtures from a better-looking male would be regarded as issuing from a higher status (the visually-pleasing plane of existence) and welcomed.

Anyhow, after his transformation Hal meets and falls almost immediately in love with Rosemary Shanahan (Gwyneth Patlrow), who, as you already know from the film's advertising blitz, is a big fatty, even though when Hal looks at her, all he sees is the ethereal Gywnnie.
Hal has a crisis, realizes the only solution is to become re-hypnotized, avoids Rosemary like the plague, and, when this solution doesn't materialize, realizes he loves her no matter what she looks like.
At the beginning of her relationship with Hal, Paltrow does a nice job expressing Rosemary's wariness at Hal's attentions, her awareness of the world's discriminations, and still, her willingness to believe that Hal might be true.

Even though we're seeing the "thin" Rosemary, the one Hal sees, her end of the canoe is slung low in the water, while Jack, at the back end, is hiked high in the air.

Since Tanya wasn't in the script, Hal had to be shown seeing some other woman with "inner ugliness." He saw a heavyset woman obnoxiously hogging one of the treadmill machines in the gym, which was exactly how the rest of the world saw her.
When Hal was kissing the maid under the mistaken impression that she was Rosemary/Kimberly, the mom called out the maid's name from around the corner and Hal managed to stop kissing her just in time before mom came into view.

Hal cannot really remember his father but his words obviously stayed with him.
The adult Hal works for a North Carolina company called JPS Funds, where colleagues counsel him that picking women based solely on looks "might not be the way to go about it." A beautiful neighbor who went out with him once no longer wants to see him and suggests he move.

When Hal comments to Rosemary's slim mother, "Wow, now I see where Rosemary gets her figure!" the discomfort of the situation is obvious, but again, the Farrellys deliciously complicate their message by later revealing that Rosemary's mother is also morbidly obese.
Shallow Hal is much more than a series of politically correct cheap shots at its audience--dismissing it as such would be as misguided as, yes, judging a book by its cover.
Shallow Hal, though it has its share of gags, is the least slapstick of the Farrellys' oeuvre; it's a deficiency more than made up for by the film's subtly incisive observation, the mark of mature comedy.

One day, Hal meets late-nite infomercial king Tony Robbins (playing himself) in an elevator, the elevator gets stuck, they get to talking, and Robbins imparts some type of hypnotic suggestion which allows Hal to see people from then on as they really are inside, rather than as they appear externally.
Since Hal is now hitting on a steady diet of fat ugly chicks, he starts to have unprecedented success with women, and he's happy because in his own perceptions he seems to be scoring with the entire Ford Modeling Agency.
