After a few years at Memorex, Shugart decided to strike out on his own, and in 1972 he gathered up some (Wealth available for investment in new or speculative enterprises) venture capital and started Shugart Associates.
Shugart went on with Finis Conner to found Shugart Technology in 1979, whose name was changed to (Click link for more info and facts about Seagate Technology) Seagate Technology in response to a legal challenge by Xerox.
In 1976, Jim Adkisson, a Shugart engineer, sat down for lunch with a customer who complained that the 8-inch drive was too big for the personal computers then emerging in the nascent (Click link for more info and facts about S-100) S-100 market.

Shugart Technology (the company formed by Alan Shugart after leaving Shugart Associates) introduced SCSI in 1979, and initially referred to it as SASI (Shugart Associates System Interface).
After a number of other companies (NCR being the first, in 1981) decided to adopt SASI, SASI received the new name "SCSI." NCR also helped start the standards process the next year, and in 1986, ANSI approved the SCSI spec (as X3.131-1986).
Before summarizing the evolution, a distinction should be made between the terminology used in the SCSI standard itself, as promulgated by the T10 committee of INCITS, and common parlance, as codified by the SCSI trade association, SCSITA.

In 1979, Shugart Associates introduced an interface called SASI (Shugart Associates System Interface).
In late 1981, NCR and Shugart agreed to merge the best features of the two solutions, and to jointly promote the concept as an ANSI standard.
After several committee meetings and after a number of other companies decided to adopt the combined standard, it received the new name "SCSI." In 1986, with SCSI already in widespread use, ANSI approved the SCSI spec (as X3.131-1986).

Shugart wanted to incorporate processors and floppy drives into complete microcomputer systems at that time, but the financial backers of the new Shugart Associates wanted him to concentrate on floppy drives only.
He quit (or was forced to quit) Shugart Associates in 1974, right before they introduced the mini-floppy (5 1/4-inch) diskette drive, which of course became the standard eventually used by personal computers, rapidly replacing the 8-inch drives.

Shugart Associates, led by storage industry pioneer Alan Shugart (who was a leader in the development of the floppy disk, and later founded Seagate Technology) created the Shugart Associates Systems Interface (SASI).
Shugart wanted to get SASI made into an ANSI standard, presumably to make it more widely-accepted in the industry.
The name was also changed to SCSI; I don't know the official reason for this, but I suspect that having Shugart Associates' name on the interface would have implied that it was proprietary and not an industry standard.
