On single and double overhead cam engines, the cams are driven by the crankshaft, via either a belt or chain called the timing belt or timing chain.
Usually, double overhead cams are used on engines with four (or more) valves per cylinder -- a single camshaft simply cannot fit enough cam lobes to actuate all of those valves.
The main reason to use double overhead cams is to allow for more intake and exhaust valves.

There are no pushrods in a Single OverHead Cam(SOHC)=4.6 engine.There are lifters,or lash adjusters,though.In an OverHead Valve(OHV)=5.0 engine the pushrods go down through the head to the lifters that ride on the cam lobes themselves.The cam is in the valley between the cylinder banks and under the intake manifold.
In OHV V-style engines, there is only one single cam in the whole engine that sits near the crank in the bottom of the V and pushes up on the pushrods which go up into the 2 heads.
The cams in the OHC engines are turned by a timing chain or a timing belt that runs all the way up from the crank (at the bottom) to the head.

With a camshaft sitting above the valve, all you have is some sort of lash adjuster (aka lifter) riding on the cam and the lifter rides on the valve.
Putting the cam in the block and actuating the valves, through rockers, with pushrods works quite well, too.
You can have four valves per cylinder with a pushrod (cam in block) engine, you just have to get fancy with the rockers and pushrods.
