Soft focus is a term used in photography and optics to describe a lens flaw.
The soft focus effect is used as an effect for glamour photography, because the effect eliminates blemishes, and in general produces a dream-like image.
The effect of a soft focus lens is sometimes approximated by the use of diffusion filter or other method, such as stretching a nylon stocking over the front of the lens, or smearing petroleum jelly on a clear filter or on the front element of the lens itself.

As I understand it, "soft focus" is where edges are sharp, but the rest is kinda blurred just a bit.
All soft focus filters try to replicate the effect of portrait lens used on view cameras up to the late 60's.
Soft focus reduces the detail in the image without giving the appearance that it is out of focus.

The basic technique is to create a duplicate layer in the image, lighten it and blur it, and combine it using a layer mode with the original.

For those of you not familiar with the terminology, front-focus means the camera is focusing slightly in front of a subject (having confirmed a focus lock); back-focus is the inverse, the camera focuses slightly behind the subject.
Two shots were taken at each aperture, using the centre focus point only in one-shot AF mode; the first relied on the camera’s AF confirmation, and the second on manual focus (I threw the lens out of focus and refocused manually to take the second shot in each set).
Minor focus inaccuracies detected at 100% won’t be visible when the picture is printed full size, especially when printed smaller than 8”x10”, something to keep in mind if your final output is going to be print.

Many think of these lenses as merely a way to "erase wrinkles", but in the right hands they can help in creating beautiful landscapes and provide a mood that strengthens other photographs.  Soft-focus lenses are tools.  Like other tools they can provide results that are good or bad.
Some Dallmeyer lenses fall into this grouping.  One type was just called the "Dallmeyer Soft Focus.  It appears to be similar to the Kodak.  Another was the "Mutac", unusual in that it was a triple convertible.  You could use the lens complete or with either of the cells by itself. 
One early device mechanically jiggled the focus control during exposure, the apparent aim being to increase depth of field.  Some early lenses were advertised as having great depth of field.  This was the same as saying that they were soft.

That said, you can use soft focus filters on digital cameras.
What I do for soft is duplicate layer then gassian blur dup layer to about 75% then back opacity back till I like it.
I've tried several brands and my favorite is Nikon's soft focus fl net diffusers and the Tallyn's.

For the finishing touches, change the Blend Mode to Soft Light, and the Opacity to 50.
I played around a bit more and added soft focus again, as well as a slight blue tint.
