Spiral fracture indicates a fracture line that spirals along the long axis of the bone; it is caused by torsional twisting or rotational forces.
Spiral fractures tend to have extremely sharp points and edges, which frequently accompany soft tissue trauma or an open fracture.
The resulting fracture will be a very long spiral with sharp points and often sharp edges.

It seems that the inspection of a fracture surface is sometimes misleading because the surface can have two distinct morphologies, which may not be due to fatigue, and a fracture which appears to have one morphology may be due to fatigue.
This is commonly observed for a brittle fracture that changes to shear for the final small section - typically in the form of a "shear lip" around the edges of the cross section.
I understand that for a fatigue failure, we sometimes find that a smooth fracture surface (sometimes with 'beach markings') is due to cyclic fatigue, and there is usually another region where catastrophic fracture has occurred when the fatigue crack has passed it's sub-critical length.

It's my experience that the biological variability and the complexity of bone as a material/structure that trying to predict fracture configurations is difficult, except in the broadest terms eg spiral fracture is a a sign of a torsional load during injury.
Fractures in life are either sudden and catastrophic events (where impact properties may have been more relevant to study) or slow progressive 'stress fracture' phenomena.
He is nearing completion of his Ph.D. thesis on the fracture mechanics of bone and will probably be able to tell you why this technique has not been applied to whole bones.
