The Constitution is silent on the issue of the secession of a state from the United States.
"Washington" is a state, a city corresponding to the District of Columbia (and thus not part of any state), and a number of cities and counties in various states.
Only two states have state capitals named for the state (however, this is a very common practice with states and provinces in other countries, where the state or province is actually often named after the capital city): Oklahoma, with capital Oklahoma City, and Indiana, with capital Indianapolis (which means Indiana City).

In a State with a smaller number of entities, the standard increment between successive place codes is larger, while in a State with a larger number of entities, the standard increment between successive place codes is smaller.
For example, Autauga County, Alabama, with a county code of 001, is assigned a FIPS 55 code of 99001 in the Alabama file; Montgomery County, Maryland, with a county code of 031, is assigned a FIPS 55 code of 99031 in the Maryland file.
The files are sequenced in numerical order based on the FIPS State codes; that is, the States and the District of Columbia occur first in alphabetical sequence, followed by Puerto Rico and the Outlying Areas in alphabetical sequence.

If the hospital or other facility fails to report in a format prescribed by the department, the department's authorized representative may access the information from the hospital or the facility and report it in the appropriate format.

For years, plumbing codes were most vocal in requiring water at temperatures no less than 140° F and even hotter for commercial and sterilization applications.
Such terminology remains within today's codes (though additional provisions have been added to the model code and some state codes to reduce the potential for scalding incidents).

New Michigan plumbing and mechanical codes took effect on Dec. 31, 2003 while new building, residential and electrical codes took effect this week.
"International codes now require a 7-3/4 inch rise and a 10 inch tread, but the Home Builders Association in Michigan has managed to keep it at the same level it has been," Behnke said.
Although the construction authority does not yet have a copy of the state codes, which is expected by early May, Behnke said that he doesn’t anticipate this causing any problem in residents obtaining permits for work to be done in their homes.

 Historic State Codes
The Edward Bennett Williams Law Library, at the Georgetown University Law Center, is carrying out a project to preserve the historic state codes printed between 1840 and 1930 for all fifty states and the District of Columbia.
There are two goals for this project: 1) to create a complete preservation collection of historic state codes from 1840-1930, and 2) to provide other libraries the opportunity to acquire these titles so that there are partial or complete preservation collections across the country.
Like other brittle books, these state codes require preservation reformatting in which contents of books are reproduced on a medium that is chemically, physically and technologically stable so that the information will continue to be accessible in several hundred years if stored under appropriate environmental conditions and used carefully.
