There are two types of stress teststests that involve exercising the patient to stress the heart (exercise cardiac stress tests) and those that chemically stimulate the heart to mimic the stress of exercise (physiologic stress testing).
The exercise stress test (also known as an electrocardiograph treadmill test) involves walking on a treadmill with progressive increases in speed and elevation (usually at three-minute intervals).
A stress test is performed to investigate coronary artery disease; determine causes of chest pain, shortness of breath or lightheadedness; determine the exercise capacity of the heart; and predict the risk of heart-related conditions such as heart attack.

The test preparation requires that the patient have nothing to eat for at least 4 hours before the scheduled time of the test.
A Stress Echo test utilizes the principle of routine treadmill testing (see section on treadmill testing) and combines it with echocardiography (cardiac ultrasound) to derive a more accurate diagnosis in patients with chest pain.

Treadmill exercise stress testing is the most basic form of stress testing which relies on the patient’s ability to walk on a treadmill while continuously monitoring the heart rate and heart rhythm.
A continuous electrocardiogram is evaluated to assess the presence of coronary ischemia (decreased perfusion of the heart arteries) during exercise.
An abnormal treadmill stress test may be followed by a second stress test with imaging capabilities (stress echo or myocardial perfusion imaging) which increases the accuracy of diagnosing coronary artery disease or may result in coronary angiography.
