The peak systolic blood pressure during supine bike exercise was higher than that during treadmill in the normal group but was statistically similar in the CAD patients.
Supine bike is associated with a lower maximal heart rate than is treadmill exercise, but has higher systolic and diastolic blood pressures.
When the patient is provided with a hip strap, hand straps to the side (not overhead), a properly positioned bike, and when the patient is given the correct instructions and is directed to control his breathing, the movements are not as destructive to the study as the literature would have you believe.

Supine bicycle stress echocardiography was 93% sensitive, 86% specific and 92% accurate for identifying patients with coronary artery disease irrespective of prior myocardial infarction or achievement of >85%% maximal predicted heart rate, The "normalcy" rate in the low probability group was 100%.
Supine bicycle stress echocardiography is a highly accurate tool for evaluating coronary artery disease, identifying both the patient with coronary artery disease and the location and extent of disease.

Prone or Supine Makes No Difference in Pediatric Lung Injury - CME Teaching Brief - MedPage Today
BOSTON, July 12-Placing children with acute lung injuries on their stomachs in the ICU has been shown to be safe and to improve oxygenation in uncontrolled trials, and some intensive care specialists have speculated that it could help improve recovery.
The study, published in the July 13 issue of the Journal of the American Medical Association, was designed to test the hypothesis that placing children in the prone position during the acute phase of treatment could improve both oxygenation and regional changes in ventilation, thereby decreasing the risk of ventilator-associated lung injury and facilitating recovery.
Children placed in the supine position had a mean of 15.8 [SD 8.5] ventilator-free days, compared with 15.6 [8.6] for kids placed in the prone position.
