Tolkien's fictional universe of Middle-earth, the Swanfleet or Sindarin Nîn-in-Eilph is a marshy area in Eriador.
Formed where the Glanduin (or "Swanfleet river") joins the Mitheithel (Greyflood), the Swanfleet is effectively an inland delta, with uncertain streams and a very uncertain difference between land and water.
Near the Swanfleet was a ford over the Glanduin, from where led a road to the ruins of Ost-in-Edhil, the ruined city of Eregion.

At its eastern end, the waters of Swanfleet merged with the River Hoarwell and formed the Greyflood.
On their way home after the War of the Ring in September of 3019, Frodo Baggins and his companions crossed the Glanduin and saw the meres and eyots of Swanfleet away to the west.

The note on Belfalas is written upside-down beginning at the bottom of the sheet (with respect to this drafting and the printing).
The Glanduin is the same river as the Swanfleet, but the names are not related.
I find on the map with corrections that are to be made for the new edition to appear at the end of this year that this river is marked by me as both Glanduin and various compounds with alph ‘swan'.

Coming to the crossing of Ost-in-Edhil they forded the still deep waters of the Swanfleet as the summer melt of the Mistys continued in the warm sun.
They made it across on good order if not a bit cold from being soaked by the icy water, but the sun soon warmed them as they rested on the north side of the river.
The soft hum and buzz of the insects in the long grass by the riverbanks was soothing, she could easily fall asleep.

Sorry - but you are wrong in thinking the Swanfleet is a river in West Rohan, or in Rohan, at all.
Swanfleet is the name given to the River Glanduin where it flows through the fens just northeast of Tharbad.
My map, one of those bookmarks that fold out into a map that were released by New Line, gives the name of swanfleet for the river.

Tolkien records the fenlands of Swanfleet within view of the Misty Mountains, a large mountain chain considered to be the backbone of Middle-earth.
The Teleri seem to have especially revered swans and build their water vessels in swan-form.

Mostly likely, the group will attempt to return back to Mathain or to Dunlending proper shortly.
