It concludes that the unauthorised take-off by the Angolan airliner from Hosea Kutako Airport, which increased the already existing stress levels of the air traffic controller on duty that day, contributed to the cause of the accident.
The probable cause of the accident was identified as having been that the Angolan flight and the Namibian aircraft were not kept adequately separated from each other as their flight paths crossed about 14,5 nautical miles north of Hosea Kutako International Airport.
Only minutes later, as the TAAG airliner was still climbing to its cruising altitude of 31 000 feet, and with the Cessna having just reached its cruising altitude of 11 500 feet, the Cessna pilot noticed the Boeing on his right on a collision course, the report states.
