Although it may be possible, I am unaware of an obvious TPN cause causing an enlarged liver.
If she can't take food orally, feeding is much better utilized thru the gut than via TPN; it would seems there's no reason she couldn't have a feeding tube placed (surgically or otherwise), and receive enteral feedings.
