Thrombotic thrombocytopenic purpura (TTP) is a clinical syndrome defined by the presence of thrombocytopenia and microangiopathic hemolytic anemia.
The damage triggers a cascade of biochemical events that ultimately leads to the characteristic feature of TTP - widespread dissemination of hyaline thrombi, composed predominantly of platelets and fibrin, which occlude the terminal arterioles and capillaries (microcirculation) of most of the major body organs, commonly, the heart, brain, kidneys, pancreas and adrenals.
The first case of TTP was described by Moschcowitz, in 1925, in which a 16 year old female was admitted to the hospital with an acute and fatal illness that included stroke and heart failure.

A new association of TTP and the drug Plavix (clopidogrel) will be reported in the New England Journal of Medicine, but because of the potential importance to patients of this association, the Journal has posted the study's results on its web site prior to publication.
Although TTP can be diagnosed with readily-available blood tests, testing of patients taking clopidogrel is not recommended, perhaps because TTP is such a rare side effect, and it is not known if earlier diagnosis (with earlier discontinuation of drug) would be beneficial.

These investigators describe 11 cases of TTP associated with clopidogrel; cases were identified by surveying hematologists, blood banks, and the drug manufacturer.
In 10 of the 11 cases, clopidogrel had been used for 2 weeks or less when TTP was diagnosed; the other case was diagnosed after nearly a year of use.
