There is an irregular narrowing of the lumen of the ileum, possible cobblestoning (fl arrowheads), and a coned cecum (white arrowheads).
These spot films of the terminal ileum show a fixed ileum displaced by mass or mesenteric retraction secondary to inflammatory disease.
More advanced disease leads to extensive ulceration of the terminal ileum and the characteristic "string sign", caused by the loss of both the normal ileal features and the cobblestone pattern.

Numerous (approximately 50) tan, sessile polyps (Figure 1), ranging from 0.2 to 0.7 cm in greatest dimension, were found closely clustered together in the terminal ileum and the ileal side of the ileocecal valve.
Sections from the ileum showed the polyps to be composed of multiple closely aggregated lymphoid follicles in the lamina propria of the ileal mucosa.

The combination of a conical cecum with a normal-appearing terminal ileum helps to rule out other entities, such as tuberculosis and Crohn's disease, in most patients, although the terminal six inches of ileum may be involved in 10% or more of patients with severe cecal amebiasis.
In both patients, there is reflux into a normal terminal ileum which permits differentiation in most patients from Crohn's disease, tuberculosis, and other inflammatory diseases of the right colon and ileocecal area.
Such pronounced involvement of the ileum is unusual in amebiasis, occurring in about 10% of patients with colonic amebiasis.

World Journal of Emergency Surgery | Full text | Nontraumatic terminal ileal perforation
Terminal ileal perforation should be suspected in all cases of peritonitis especially in developing countries and surgical treatment should be optimized taking various accounts like etiology, delay in surgery and operative findings into consideration to reduce the incidence of deadly complications like fecal fistula.
Perforation of terminal ileum is a cause for obscure peritonitis, heralded by exacerbation of abdominal pain associated with tenderness, rigidity and guarding, most pronounced over right iliac fossa.
However in the patients where the terminal ileum is grossly inflamed with multiple perforations, perforation-operation delay >48 hours, fecaloid peritonitis; something more than mere closure of perforation needs to be done and end to side ileotransverse anastomosis is a better procedure.

At surgery, the hepatic flexure of the ascending colon was found to be completely obstructed by a hard concentric mass, and about 15 cm of the terminal ileum, just proximal to the ileocecal valve, was markedly dilated and diffusely thickened.
A cut section of the thickened terminal ileum revealed marked submucosal edema to a depth of approximately 10 mm, though there was no evidence of mucosal lesion.

It is by virtue of alphabetization rather than contribution that Crohn's name appeared as first author: because this was the first time the condition was reported in a widely-read journal, and the disease has come to be known as Crohn's disease for reasons of publicity rather than precedence.

Terminal segments of rabbit ileum retain spontaneous contractile activity when isolated and maintained in physiological solution.
It has been suggested that the ability of compounds to inhibit this spontaneous activity may be related to their irritant effect, thus the preparation may provide an alternative means of assessing likely potency of irritants in the eye in the in vivo situation.
Sections of terminal rabbit ileum are removed, suspended in an organ bath and the spontaneous contractile activity monitored.
