Finally, pursued by his long-time lady love among the Q, he is forced to tell Janeway the truth: he wants to have a child with her to try to put an end to a devastating civil war among the Q which is causing the supernovas.
Q was less superficially misogynistic than in "Death Wish," and more sympathetic - for once he looked less like a bully than like a child overcompensating for his own limitations.
In the end, Q's committment is clearly to his child and not to his mate, while Janeway gets asked to play doting godmother for the baby which, as she lamented, she probably won't ever be able to have herself.

Q also explains that the acts of Quinn (a Q encountered by Voyager before) have brought unrest to the Continuum, with two main sides arising – the ones who wish for individuality and separatism, and the others who want the status quo.

Q says the Q have always existed, yet his girlfriend (I’d call her “Qute”, but that might be dangerous) says she’s 5 billion years old.
When Q is wounded in the shoulder and Janeway helps him out of the house, she grabs him by that shoulder (the left one, I think) and puts his left arm over her shoulder in an effort to support Q. He does not appear to be in any pain from the rough handling.

John de Lancie's Q and Kate Mulgrew's Janeway do have a surprising amount of chemistry and work well together on screen, especially when it comes to putting across those delightful Q style comedic moments.
That said, this episode becomes all too predictable once the female Q played by Suzie Plakson, who does a fine job nevertheless, appears on the scene.
Still despite the fact it never quite gels or works as well as it should, I can't seem to hold it against this one too much because it did make for a fun and at times surprisingly moving viewing experience.
