Thelarche is usually noticed as a firm, tender lump directly under the center of the nipple (papilla and areola).
Thelarche is also referred to as a "breast bud", or more formally as Tanner stage 2 breast development (Tanner stage 1 being the entirely undeveloped prepubertal state).
Thelarche is usually the first physical change of puberty in girls, and is a result of rising levels of estradiol.

Thelarche means "the beginning of breast development." Therefore, if a girl begins to show breast enlargement at an early age (anywhere from birth to six years), it is called "premature thelarche."
Premature thelarche is not a disease; instead, it is a normal finding in some young girls or female infants.
Most commonly, premature thelarche is diagnosed in a female infant or a girl up to three years of age.

"Thelarche pathway" linked to obesity, possibly breast cancer - News
Girls who begin puberty with breast development before pubic hair growth are at increased risk for obesity and possibly also breast cancer, according to a report published in the June issue of the Journal of Pediatrics.
Therefore, in the current study, girls in the thelarche pathway had a number of factors that could increase their risk of breast cancer--earlier age of menarche, greater BMI, and increased body fat.
As the study included only white girls, "it is unclear whether the impact of asynchronous maturation is the same in fl females as it is in white female," the authors note.

In premature thelarche, there are no other signs of early puberty, except the breast development.
Close medical followup of the girls with this condition is needed, every 6 months, because a small percentage of these girls will additionally have precocious puberty (early puberty) before the age of 8 years, and precocious puberty is abnormal.
No laboratory test is available to predict which girls with premature thelarche will go on to have precocious puberty.

When menarche has failed to occur for more than 3 years after thelarche, or past 14.5 years of age, the delay is referred to as primary amenorrhea.
