As i know after read the article about Therac 25, I found that after therac -25 was introduced, there are six known accidents involved massive overdoses by the Therac-25 -- with resultant deaths and serious injuries between June 1985 and January 1987.
For example the software for the therac 6 and therac 20 are no suitable for the therac 25.
