Abstract: Torsades de pointes is a polymorphic form of ventricular tachycardia that is usually associated with prolongation of the QT interval.
In torsade de pointes associated with the hereditary long QT interval syndromes, early diagnosis leading to treatments designed to both shorten the QT interval and block the beta-adrenergic-induced instability of the QT interval is essential.
Torsades de pointes spontaneously progressed to ventricular tachycardia with a uniform morphology in three patients and to ventricular fibrillation in four.

Torsades de Pointes in Patients Receiving Terfenadine or Astemizole.
Torsades de pointes is a form of polymorphic ventricular tachycardia that is preceded by a prolongation of the QT interval (Monahan et al, 1990).
Torsades de pointes occurred in one patient whose daily dosage had been increased steadily over six weeks to 360 mg daily; this was the first reported case of torsades de pointes caused by an enhanced therapeutic dose of terfenadine in isolation (MacConnell and Stanners, 1991).

The patient experienced angina pectoris followed by episodes of torsades de pointes, which were interpreted as the result of bradycardia, and the bradycardia as the result of high grade AV block induced by increased vagal tone caused by ischaemia in the presence of digoxin intoxication (serum digoxin was 2.5 micrograms/l).
In drug-induced torsade de pointe the QT interval was moderately prolonged.
Torsade de pointes is most commonly precipitated by QT prolonging drugs, mainly type IA antiarrhythmic therapy such as quinidine and disopyramide, and other antiarrhythmic agents are reported to cause torsade de pointes as well.

Torsades de pointes is a potentially life-threatening polymorphic ventricular tachycardia that is characterized on a surface ECC by a twisting QRS morphology that is associated with prolongation of the QTc interval.
Although the precise mechanism of torsades de pointes remains unknown, substantial evidence suggests that the arrhythmia is provoked by the initiation of early afterdepolarizations.
Therefore, the purpose of this study was to determine the effect of IV haloperidol on these parameters that are known to be associated with torsades de pointes.

 Atrial Fibrillation Foundation
Torsades de pointes (or torsades) is a lower chamber (ventricular) heart rhythm disturbance that can cause dizziness, fainting, and a potentially fatal cardiac arrest or sudden death.
Torsades may be provoked by certain heart rhythm medications as well as other medications (for a complete list, see www.qtdrugs.org).
The risk of torsades can be minimized by selecting appropriate drugs for the individual patients, making sure that the body chemistry is corrected as much as possible, and by initiating medications in a closely supervised hospital setting in patients with a particular risk of this disorder.

Sotalol, like some antiarrhythmic agents, has been associated with a specific form of arrhythmia, torsades de pointes, which is defined as a polymorphic ventricular tachycardia with prolongation of the QT interval and QRS complexes of changing amplitude that appear to twist around the isoelectric axis.
Torsades de pointes is dose dependent, usually occurs early after initiating therapy or escalation of the dose, and terminates spontaneously in the majority of patients.
In patients with a history of sustained ventricular tachycardia, the incidence of torsades de pointes was 4%, and worsened VT was approximately 1%; in patients with other, less serious, ventricular arrhythmias and supraventricular arrhythmias, the incidence of torsades de pointes was 1% and 1.4%, respectively.

Because the risk of torsades de pointes is sufficiently high at typical clinical dosages of sotalol, dofetilide, and ibutilide, the review author recommends in-hospital cardiac monitoring when therapy with these agents is initiated.
The risk of torsades de pointes is not related linearly to the degree of QT prolongation, although any drug that prolongs the QT interval beyond 500 msec is thought to confer an elevated risk.

The worldwide reporting of torsades de pointes (TdP) associated with the use of terfenadine and astemizole has prompted the Therapeutic Products Directorate to change the status of these drugs from nonprescription to prescription.
TdP may also result from the use of drugs causing QT prolongation in patients with medical conditions such as hepatic dysfunction or congenital long QT syndrome or in those with electrolyte disturbances (particularly hypokalemia and hypomagnesemia).
Because the list of drugs causing QT prolongation and TdP is continually increasing, we encourage health care professionals to report suspected adverse drug reactions to the Adverse Drug Reaction (ADR) Reporting Unit (fax 613 957-0335) or to their regional ADR reporting centre.
