United Airlines Flight 232 - Wikipedia, the free encyclopedia
United Airlines flight 232 "UA232", "UAL232" (United 232 Heavy) was a scheduled flight operated by United Airlines.
The flight took off at 14:09 (CDT) from Stapleton International Airport, Denver, Colorado and was due to have flown to Philadelphia International Airport in Philadelphia, Pennsylvania via O'Hare International Airport in Chicago, Illinois.
Eventually the flight crew figured out that the only way to control the plane was by adjusting the thrust on the two remaining wing-mounted engines.

One was this: the captain of the flight, I had felt a responsibility for the accident, but that's this they had to convince us that, really, there wasn't a lot more that we could have done in the airplane to do it, and it took some time, because we all had guilt complexes about this.

The Crash of United Flight 232 by Capt. Al Haynes
I think everyone probably remembers that on July 19, 1989, United Airlines flight 232 departed Denver at about 2:09 PM, climbed uneventfully to a cruise altitude of 37,000', and at approximately 3:16, the flight notified ATC center that the #2 engine had failed.

 The Ingenuity Gap
At first, United 232's experience seems to be no more than an isolated, harrowing event in the skies of the United States - a tale of heroism in the face of terror, of discipline and skill in the face of unforseen catastrophe.
Yet the flight of United 232 is more than a vivid metaphor for a world of converging complexities and connections, of decision making at high speed under high uncertainty, and of the difficulty of managing in such circumstances.
And 2,170 flight hours later, in a split second, the crack shot outwards to the edge of the disk, and the disk blew apart.

"Flight attendants are required to secure all items onboard an aircraft to comply with FAA regulations, including carry on baggage and coffee pots, but not infants and small children," Lohr said.
If the FAA and the airlines continue to allow children under two to be held on a parent's lap, many parents will continue to have the false impression that this practice is safe.
"When preparing the aircraft cabin for an emergency, flight attendants should not have to look a parent in the eye and instruct them to continue to hold a child on their lap when we know there is a very real possibility that child may not survive without proper restraints," Lohr said.
