Thomas Mathew M.The “Tongue Sign” of Uraemia. The Calicut Medical Journal
In 92% of cases with uraemia, the tongue was large, macroglossic, pale and had very characteristic, striking and prominent impressions of the teeth all around the edges (Fig.1).
The changes observed in the tongue in uraemia could very well be due to the acidic pH of saliva and or due to the abnormal oral bacterial flora as well as the high urea content of the saliva in the uraemic patients.
It could be postulated that it is the “uraemic milieu” that has produced the changes in the tongue and when it is corrected by dialysis or transplantation or by recovery from acute renal failure, the tongue regained its normal colour, size and shape.

Effects of chronic uraemia on the formation of glucose and urea plus ammonia from L-alanine, L-glutamine and L-serine in isolated rat hepatocytes. The effects of chronic uraemia on glucose production and nitrogen release (urea plus ammonia formation) from alanine, glutamine or serine in isolated rat hepatocytes were studied.
Uraemia increased the rate of formation of urea plus ammonia from all three amino acids by 38-93% when they were present at a final concentration of 10 mmol/l.

In lupus patients, anaemia can be caused by chronic inflammation, iron deficiency, prolonged uraemia, or haemolytic anaemia.
Uraemia is marked kidney insufficiency in which wastes normally excreted by the kidneys remains in the bloodstream.
Uraemia can cause anaemia in lupus patients, but can be treated with androgens (male hormones), or with a hormone called erythropoietin.

Although it would seem to be logical to institute dialysis before the development of overt uraemia or malnutrition, which are common at renal GFRs between 5 and 10 mL/minute/1.73 m2 (Obrador and Pereira 1998), the risks of early start dialysis need to be considered.
These include peritoneal dialysis complications (eg peritonitis etc), premature exhaustion of an important dialysis modality that may be more useful later in the natural history of ESRD (Hiroshige et al 1996, Hufnagel et al 1999), potential acceleration of the loss of residual renal function and increased costs.
If there is no evidence of uraemia or malnutrition, increase the frequency of observation to monthly and recommend dialysis when indicated (uraemia or malnutrition).

They said the itchiness was from that b/c my uraemia levels were out of control.
Unfortunately, uraemia is more likely on haemodialysis, just because you have that extra time between dialysis sessions, where both your weight and your blood figures generally have time to rise.
Possibly, the itching due to the uraemia could be because you are not being adequately dialysed and your figures and not coming down far enough towards normal values following a dialysis session.

London is in a state of uraemia following an error in diet, causing a faulty elimination of the kidneys.
Uraemia is a condition of blood poisoning which occurs when the kidneys fail to function properly, as in Bright's disease and fail to excrete urea which is retained in the blood and upsets the nervous system, causing drowsiness, headaches, giddiness and in extreme causes coma.

 Amyloidosis (dog)
The first organ to become seriously affected is the kidney so the observed clinical signs are usually those of chronic renal failure, other affected organs include the liver, spleen and the intestinal tract.
The deposition in the kidney results in protein leakage into the urine, as the problem progresses the destruction of the kidney nephrons leads to chronic renal failure and uraemia.
Weight loss as a result of the protein loss may be the first indication of illness, as the uraemia develops then lethargy, anorexia and vomiting occur, increased thirst and urination are common.

CIN'2003. Tattersall. When to start dialysis
It is theoretically possible that some of the mortality in dialysis (particularly the early mortality) may be related to preventable factors including uraemia and malnutrition before dialysis starts.
By starting dialysis earlier, some of this initial uraemia and malnutrition may be avoided and there is the prospect that mortality may be reduced.
Creatinine is generated from muscles and its generation rate is determined by muscle mass, activity and, to a lesser extent, dietary protein intake.

This study was conducted to investigate the prevention of these uraemia-induced vascular abnormalities by the angiotensin-converting enzyme inhibitor (ACE-I) Ramipril.
These results suggest that these specific vascular abnormalities in uraemia may be mediated by renin or bradykinin, or by the direct action of angiotensin II on vascular smooth muscle.

No investigation has been undertaken to evaluate the AA levels in plasma, muscle and red blood cells (RBC) simultaneously, although the RBC pool of free AA constitutes a large proportion of the free AA in blood, and RBC are involved in the interorgan transport of AA.
Moreover, in most of the earlier studies reporting RBC AA levels in different clinical conditions, including uraemia, AA were determined in whole blood, which also includes AA from white blood cells and platelets.
The RBC and muscle glutamate relationships to the IGF-I/IGFBP-1 axis and muscle ASP/DNA ratio support the proposal that RBC glutamate or RBC glutamine/glutamate ratio may be used as markers of catabolism, and that changes in the bioavailability of IGFs are linked to the regulation of glutamate distribution.
