The purpose of this study was to use echocardiography as a means to examine cardiac dimensions and beat to beat changes in response to the Valsalva manoeuvre in twelve normal healthy female volunteers.
It was found that at the end of the strain phase of the Valsalva manoeuvre, left ventricular dimensions had decreased by 13.6% (+5.19%) for end-diastole and a corresponding 11.1% (+6.78%) for end-systole.
The conclusions drawn from the data are that echocardiography successfully demonstrated the effects of the Valsalva manoeuvre on left ventricular dimensions and heart rate responses.

I have asked the Valsalva manouuvre numerous times in Practive Vivas and it is extremely unusual for the registrar to reach the stage you might think about asking these difficult questions.
Also, a Valsalva occurs during Anaesthesia either on purpose (eg a neurosurgeon may request it to check haemostasis) or accidentally (forget to open expiratory valve when patient changed from ventilator to spon resps and rebreathing bag becomes quite balloon-like).
