The chest voice is the register typically used in everyday speech.
The middle voice, also known as the "blend" or "mix," is the term used in the Bel Canto Opera singing method and Speech Level Singing to describe the range of notes which marks the crossover between the chest and head registers.
Head voice is different from falsetto in that it is possible to connect it to the chest voice.

The falsetto register is used by male countertenors to approximate the register in classical voice that previously employed castrati, in pieces written before castratism became socially unacceptable and eventually universally outlawed.

Singing in the middle register is the ability to sing smoothly in the area where a natural break occurs between chest to head voice (ie: from a heavy to a lighter voice)...
The mixed voice (the ability to blend one's registers so that there are no transitions from your chest to head voice) is then really created when a singer sings throughout their range, or more specifically in that zone of transition where a vocal bridge would normally happen.
