Decker wasn't the First Officer (although we never found out who held the title on an Enterprise run by Captain Willard Decker) and presumably the ship, once taken over by Kirk had a captain, a first officer and a temporary executive officer.
I think it likely the First Officer position was vacant and when Decker was moved down to "number two" when Kirk assumed command, he filled the first officer/executive officer billet by default, hence the phraseology.
Perhaps it's not used by today's navy, but in many business circles it can mean a very high ranking person without specific duties that directly effect the day-to-day running.
