Failure analysis techniques for semiconductors and other devices
The infant mortality failures are due to defects during manufacture, improper design or implementation of the component, or freak failures that have not failed during screening.
Examples of wear-out failure are: corrosion, electrical leakage, insulation breakdown, migration of metallic ions in the direction of current flow, cracking of the encapsulating material due to deterioration of the material, and cracks in the bond wires due to repeated stresses.
Investigative procedures To initiate a proper corrective action, the failure mechanism of the component must be correlated to the field observations of the operating conditions that caused the failure.

Double switching - Wikipedia, the free encyclopedia
Double switching is the practice in railway signalling in particular of cutting the power to a relay in both the positive and negative sides, so that a single false feed of current to that relay is unlikely to cause a wrong side failure.
A loose wire bypassed the contacts by which the train detection relays switched the signal, allowing the signal to show green when in fact there was a stationary train ahead.
Failure of the insulation of the wiring in that circuit allowed the baggage door to be unlocked by a false feed, leading to a catastrophic de-pressurisation.

 Modamag.com |Wrong Turn (DVD Review)
It’s just that “Wrong Turn” is a throwback to the kinds of horror-in-the-forest films seen in the 1970s and early 80s, like “The Final Terror” or “Madman”, and that isn’t going to earn her the kind of respect she deserves as an actress.
Fortunately, “Wrong Turn” provides a couple of solid scares, some tense moments of grotesque brilliance and a decent leading cast that makes the film better than it probably should be.
Side 2 has the preferred Widescreen version of the film, the same commentary, a cool poster concept gallery, 3 deleted scenes (one of which is a series of dailies), a trailer and one final featurette; “Fresh Meat: The Wounds of Wrong Turn” (9 ΒΌ min).

Both sides made what concessions they felt they could but were unable to agree on the final status of Jerusalem and various religious sites and on the right of Palestinian refugees to return to Israel.
When the principals returned home, both were greeted by mobs of people saying that their side had conceded too much and, as a result, the Israeli government was unable to maintain its majority in the Knesset.
Since a consolidation of territory—so that on one side of the line it is all Palestinian and on the other all Israeli—would require each side to abandon settlements to the other, it doesn't seem that a deal can be struck in this direction either.

Heart failure (sometimes known as congestive heart failure [CHF]) is a serious condition in which the heart is not pumping well enough.
Depending upon the nature of the underlying damage or malfunction that leads to heart failure, medications may be prescribed to reduce the heart’s workload, affect remodeling, counter abnormal hormonal levels, increase blood flow, widen vessels or eliminate excess water from the body.
It is important to note that when heart failure is a result of underlying damage or decreased blood flow due to blocked arteries or high blood pressure (hypertension), the goal of treatment is usually to relieve the symptoms of these conditions.

The physical symptoms of heart failure, combined with the depression and anxiety that it can sometimes provoke, may make you feel weak and vulnerable, as if you've lost control of your life.
A particular concern for people with heart failure is a connection between this heart problem and sleep apnea, a disorder in which a person temporarily stops breathing for ten seconds or more while asleep.

 Byzantine failure Summary
Generally, fail-stop failures (failures where a component simply stops functioning) are considered the easiest to deal with.
In fault-tolerant distributed computing, a Byzantine failure is an arbitrary fault that occurs during the execution of an algorithm by a distributed system.
It encompasses those faults that are commonly referred to as "crash failures" and "send and omission failures." When a Byzantine failure has occurred, the system may respond in any unpredictable way, unless it is designed to have Byzantine fault tolerance.

The Anabolic Steroid Control Act: The Wrong Prescription by Rick Collins, Esq.
Despite a virtually one-sided presentation in the lay press, the position that anabolic steroids are such dangerous substances as to warrant militaristic government enforcement tactics is surprisingly controversial.
Against criminalization are arguments that such penalties have proven to be a failure in stemming abuse of other drugs and alcohol, that criminalization only increases the underground fl market, and that efforts are best confined to education and rehabilitation.
Since some of the greatest dangers inherent in self-administered steroid use involve the failure to be monitored by a doctor, the Control Act has succeeded in greatly escalating this danger and has created an even wider gap between the users and the medical community.

Distant signal is approximately 600 yards on the down side of the ’box, and gives the driver information on whether if he should expect to stop at the home signal just outside the ’box.
The starting signal is another 200 yards or so on the up side of the box, and is the one which indicates if the next section (between the starting signal and the next signalbox) is clear.
This is one of the advantages of modern power-signalboxes, the fact that signals are independent of the signalbox and may be put in automatic mode where the aspect simply follows the previous signal unless a train is in the section behind the signal, i.e., the section protected by that signal.
