I have some questions: When you ask, in question 2, about the "run chart in control," are you talking about the R chart in an XbarR chart?
If you are, the scenario you describe could be caused by subgrouping unlike things in the x chart.
You could exclude them all, but until you know what is causing them, you will be unlikely to improve your process, and any limits you construct will not necessarily reflect the underlying process.

You would typically choose an I-MR Chart when you have variable data (which it sounds like you have) and your taking single readings (measurements) because there is no rational subgroups, or no way to form rational subgroups with your process/data.
Use the IMR charts when using continuous data with a subgroup size of n=1.
When subgroup sizes are varied and/or their frequency is varied (the two are independent), the IMR chart is actually more sensitive and appropriate.
