Xyphoid access for cardiac surgical procedures - Patent 6199556
Before finally closing the xyphoid incision, a scope is used to check the anastomosis for kinking or bleeding, to check the position of the drainage tube, and to check the integrity of the pleura.
To retract and offset the xyphoid access incision, the retractor arm 12 is inserted in the xyphoid incision to engage the lowermost edge of the lower rib.
3, the xyphoid incision may bisect a cartilagenous portion of the xyphoid appendage; placement of the retractor arm 12 and lifting arm 13 adjacent to, and on either side of, the xyphoid maximizes the utility of the offset function, and provides an ideal surgical field.

Armor of this segment may be manifested by epigastric spasm, muscular tension, and tenderness, especially along the xyphoid process, the margin of the lower ribs dorsally to the lumbocostal arches, and a particular respiratory disturbance which consists of a ballooning of the abdomen on expiration indicating that the diaphragm is fixed in the descending position.
Perhaps the distinction is determined by the locus of the anorgonotic process in the biosystem.

Method and apparatus for permanent epicardial pacing or drainage of pericardial fluid and pericardial biopsy - Patent ...
Conventional techniques for pericardial biopsy and drainage of pericardial fluid include a left thoracotomy, and direct attack on the left side of the pericardium, through the thoracotomy incision, for conventional biopsy and drainage of the fluid; the left chest cavity then being drained via a chest tube.
The second conventional approach is excision of the xiphoid process through a larger, midline incision and direct biopsy of the pericardium after grasping the pericardium with a surgical clamp.
The epicardial pacing process within which the pericardioscope is utilized is for the insertion of a pacemaker and the corresponding drainage of pericardial effusions.

With the patient supine, the therapist locates the xiphoid process via palpation and measures the circumferences of the chest, with a soft tape measure, at both the xiphoid process and the axilla.
Chest expansion in the normal adult is 3 ¼ inches, plus or minus ¼ inch, when measured at the axilla, and the chest and epigastric region rise simultaneously, due to the normal functioning of the diaphragm and intercostals.
Patients with upper thoracic injuries may have larger chest expansion at the axilla than at the xyphoid process because only the upper intercostals are functional.

For lumbar fusions (exclusive of fusion for fracture or tumor), the TLSO trimline can terminate at the xyphoid process or just below the breast lien and this will provide immobilization up to T-11.
The primary role of a postoperative orthosis is to protect the surgical spinal implant from undue loads during the process of fusion.
The segmental motion limiting requirements of a postoperative orthosis are not as stringent as in the case of non operative treatment since the orthosis is only being used as an adjunct to protect the implant from loads and stresses.
